Free & Subsidized Clinic List - A Resource Directory

The Free and Subsidized Clinic List serves as an access resource directory of free and low-cost healthcare service for the uninsured and under-insured in Polk County and central Iowa. It is designed to be used by healthcare coordinators, providers, educators, residents, and others, to assist in timely access to care.

Originating with Healthy Polk 2000, now maintained by HAP, the List continues to function as an up-to-date resource database. Healthcare services for the uninsured and under-insured are "dynamic" in that changes in clinic hours, location and focus occur on a regular basis in response to community need. Semi-annual updates to the List are solicited by HAP and providers are encouraged to provide updates as they occur.

Utilize the search capabilities of this document to quickly find the information you need. Each listing includes the contact information, hours of service, appointment necessity, interpreter availability, services, and applicable cost.

Services include well-baby, immunizations, prescription, basic, dental, behavioral, women's, and reproductive health, physicals, screenings for pregnancy, lead, blood sugar, hemoglobin, cholesterol, blood pressure, foot health, chemical dependency, STD's, and HIV, sexual assault exams, wellness workshops and diabetes support groups. Referrals are made for food, shelter, clothing, and transportation to healthcare appointments. Application assistance is available for general assistance and special programs.
