MISSION: The Health Access Partnership (HAP) is a collaborating group of individuals and organizations committed to assuring 100% access to quality health care in Polk County.

VISION: HAP is committed to create an integrated, comprehensive, coordinated health care system ensuring access for the uninsured of Polk County.

Current HAP Initiatives Include:

  • Neighborhood Health Initiative
  • Child Friendly Work Group
  • Cultural Fluency Work Group
  • Healthcare Coverage for Kids Coalition
  • Healthy Communities Access Program

MAY 2006 - COVER THE UNINSURED WEEK

This year, Des Moines has been chosen as one of nine target markets for the Cover the Uninsured Week campaign. May 1-7 has been designated as Cover the Uninsured week, Iowa's events will include a kick-off press conference, interfaith events, health screening and enrollment fairs, a business summit and a campus event. FEBRUARY 2006 - HAP'S PRIMARY CARE SAFETY NET INTEGRATION STRATEGY FOR POLK COUNTY

In July 2005, HAP retained the Iowa/Nebraska Primary Care Association to conduct a short-term planning process. This was designed to assist the HAP Board in establishing a direction for future planning and is articulated in the following reports.
